Home Hospital Services
Overview
Home/Hospital Services (H/H) provide tutoring for students who are temporarily unable to attend school because of a health condition. The goal is to help students keep up with core academic subjects and make a smooth transition back to school. Tutoring will be for General Education classes only.
Who Qualifies for Services?
Students may qualify if:
-
A licensed medical or mental health professional confirms a physical or mental health condition that prevents school attendance.
-
The absence is expected to last at least four weeks (total, not necessarily consecutive).
-
The student is enrolled full time, general education classes, in a Puyallup School District school.
Note: Eligibility is determined by the Student Support Director and Home Hospital Liaison after receiving the completed Home/Hospital Referral Form.
How to Apply
-
Contact your school counselor or nurse. They will help you start the referral process.
