Home Hospital Services
Overview
Home/Hospital Services (H/H) provide tutoring for students who are temporarily unable to attend school because of a health condition. The goal is to help students keep up with core academic subjects and make a smooth transition back to school.
Who Qualifies for Services?
Students may qualify if:

A licensed medical or mental health professional confirms a physical or mental health condition that prevents school attendance.

The absence is expected to last at least four weeks (total, not necessarily consecutive).

The student is enrolled full time in a Puyallup School District school.
Note: Eligibility is determined by Special Services after receiving the completed Home/Hospital Referral Form.
How to Apply

Contact your school counselor or nurse. They will help you start the referral process.

Submit the required medical documentation signed by a qualified medical provider.

Special Services will review eligibility and contact families once approved.
Services begin when Special Services receives the signed medical statement, and end the day the student returns to school.
Responsibilities During Services
- Student
- Work at least 3 hours a day on assignments
- Complete assignments in a timely manner
- Be prepared to work at scheduled tutoring sessions
- Ask for help or clarification
- Parent/Guardian
- Have an adult in the home during scheduled tutoring sessions
- Provide a clean, quiet work area
- Support the student in meeting responsibilities
- Communicate with the school when the student will return to school
- Complete and return the Home/Hospital feedback form to Special Services
- Student's School
- Provide assignments and materials promptly, with focus on core subjects.
- Accommodate and modify assignments as needed
- Correct and grade assignments
- Tutor
- Arrange with parents for two 2-hour sessions per week
- Contact school staff about class assignments
- Gather and deliver assignments to the student
- Provide assistance and guidance to the student
- Provide feedback and return completed work to teachers
- Consult with parents on the student’s progress
- Inform Special Services when the student returns to school
Intermittent or Extended Absences
- H/H services may be available for students with chronic or intermittent medical conditions.
- Services are provided for absences lasting a week or more at a time if eligibility requirements are met.
- Tutoring is not provided during school vacations unless the student is enrolled in a district summer school program.
Transitioning Back to School
When a student is ready to return:

The parent or guardian should notify the school as soon as possible.

The tutor and school staff will coordinate a smooth transition back to the classroom.
