The Food Service Department is pleased to introduce My School Menus powered by Health E Pro—a mobile app with interactive menus, allowing easy access to complete nutrition information for the school meal.

Nutrition Information

The Allergy and Asthma Foundation reports the most common foods to create an allergic reaction are milk, soy, eggs, wheat, peanuts, tree nuts, fish and shellfish.  Puyallup School District food service does not serve any items containing shellfish.  All items that contain nuts are noted on published menus.  Since milk, soy, eggs, and wheat are common components within many products; we will make every effort to help inform students of specific foods whose ingredients may pose them problems. 

Those with allergy concerns may inquire about meal accommodations or request copies of nutrient labels from the Food and Nutrition office by calling (253) 841-8773

Student Menu Modification Procedures

To request a Menu Modification for your student, a recognized medical authority must complete a "Request for Menu Modification" form which must:

  • Be medically necessary
  • Be signed by a state recognized medical authority
  • Indicate foods to be omitted/avoided from the student's diet
  • Describe how ingestion of the food impacts the student
  • Specifies the food(s) to be substituted/served instead

To file a program discrimination complaint, a Complainant should complete a Form AD-3027, USDA Program Discrimination Complaint Form which can be obtained  from any USDA office, by calling (866) 632-9992, or by writing a letter addressed to USDA. The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation.
