Washington State Healthy Youth Survey: October 6-24, 2025

The Puyallup School District will soon participate in the Washington State Healthy Youth Survey (HYS) for students in grades 6, 8, 10, and 12.

This survey is conducted statewide every two years and provides valuable data on the health and well-being of our students. Your child's school will administer the survey between October 6-24, and students will have one class period (approximately 50 minutes) to complete it. 

Survey Key Points

  • The survey helps parents, schools, and communities identify and address issues impacting our youth, by tracking trends over time. 
  • Schools and districts receive reports of their results, enabling them to better understand and meet student needs. 
  • PSD joins approximately 200,000 students in grades 6-12 across the state who take the survey every two years.

Questionnaire Topics

The HYS questionnaires cover various topics, including demographics, substance use (alcohol, marijuana, tobacco, vaping, and other drugs), health concerns (nutrition, fitness, health conditions, mental health), school climate, hope, and risk and protective factors (community, school, peers, family). 

Participation in the survey is voluntary and anonymous. If you prefer to opt your child out of taking the HYS, please contact your child's school directly before October 6.
