Note: New or non-active applications are first-time request. All required documents must be uploaded in the application.

Renewal applicant or recently moved outside the PSD boundaries (for current or next school year)

Note: You will only submit this form if you currently have an active student in the Puyallup School District. You will be redirected to the OSPI Website to complete a Choice Transfer Request.

Timeline for Out-of-District Waivers: 2026-27 School Year

• Elementary - Application window opens February 14, 2026
  • Applications submitted by February 28 will be reviewed by April 15.
  • Applications submitted after March 1 will be reviewed on or after May 1.
  • Applications submitted after June 15 will be reviewed on or after the 4th day of school.
     
• Secondary (Grades 7-12) - Application window opens December 10, 2025
  • Applications received by January 9 will be reviewed on or after February 28.
  • Applications submitted after January 10 will be reviewed on or after April 10.
  • Applications submitted after June 1 will be reviewed on or after the 4th day of school.
