Testing Dates/Times

OSS Physical Therapy has set up convenient times to provide baseline concussion screens for Puyallup School District Athletes. Proof of baseline concussion testing is required prior to participation in basketball, football, soccer, volleyball, and wrestling, and are valid for two full years. Baseline concussion testing is by appointment only.

Times are set up to accommodate the highest volume of athletes in the most efficient way possible. You can expect the overall time for the baseline to take 15-25 minutes.

Cost of screening: $15 dollars payable by cash, check or charge. (checks payable to OSS Physical Therapy)

On the web link you will be asked for your preferred testing times. You can expect an email reply within two-three business days confirming your scheduled time and any follow up instructions.
*Please DO NOT call the clinic to schedule an appointment*
**We will NOT be accepting walk-in baselines**

Students are able to get a BCS from their medical provider. The benefit of a BCS at Olympic Sports & Spine is the cost is lower than a co-pay, and the OSS trainers have access to the BCS in the event of a suspected concussion.
